Post Release Death in Custody Investigation Reports

 

In line with our Terms of Reference for Investigation of Deaths in Prison Custody, the Prisoner Ombudsman has discretion to investigate, to the extent appropriate, deaths that occur within fourteen days of a person being released from prison where issues have been raised relating to the care the person received in prison. In exceptional circumstances, at a family’s request, the Prisoner Ombudsman can also consider other deaths that fall outside this time period, where issues have been raised relating to the care received in prison.
